A tutorial was requested for this icon: 
Made with Paintshop Pro 8, but I believe it's translatable to Photoshop and other versions of PSP. Basic knowledge required, but if you're unsure of how to do something, just ask and I'd be happy to help.
1. Started with this screencap, courtesy of
_jems_. Cropped, resized to 100x100, sharpened [adjust>sharpeness>sharpen].

2. Duplicated the base and set it to screen. Duplicated that layer 4 times, resulting in 1 base layer underneath 5 screened layers of the base.

3. Duplicated the top screened layer of the base and changed the setting to soft light. Then, desaturated it [adjust>hue and saturation>hue/saturation/lightness and then change saturation to -100].

4. Between the top layer set at soft light and the first screened layer, created a new layer. Set the layer to exclusion and then filled it with a dark blue [#0b1837].

5. On top of the soft light layer, created a new layer. Filled it with a mid-brown gradient [there isn't much deviation between the browns in the gradient, so a mid-brown fill would work just as well] and set the layer to multiply.

6. Created a new layer on top of the brown multiply layer. Filled it with a light blue gradient and set the layer to soft light.

7. Created a new layer on top of the soft light layer. Filled it with the same brown gradient as before and set the layer to multiply again.

8. Duplicated the brown multiply layer, kept the layer at multiply, changed the opacity to 50.

9. Created a new layer on top of the new multiply layers. Added text [Century Gothic, pt. 1, black, auto kern]. I don't remember what I put for the text, but it's only meant to be decoration. Set the text layer to soft light.
